Telehealth Informed Consent
By requesting or participating in telehealth services facilitated through Atlas HRT, you acknowledge and agree to the following. Effective date: September 1, 2026.
Telehealth Services
Telehealth allows healthcare services to be provided through electronic communications and technology rather than exclusively through an in-person visit. Atlas HRT facilitates access to independently licensed healthcare providers who may provide telehealth evaluations and related medical services.
Independent Healthcare Providers
You understand that the healthcare provider responsible for your medical evaluation is an independent licensed healthcare professional. Atlas HRT does not independently diagnose medical conditions, prescribe medications, determine medical treatment, or direct the clinical judgment of independent healthcare providers. The healthcare provider is solely responsible for determining whether telehealth is appropriate, whether additional evaluation or testing is required, and whether treatment or prescription medication is medically appropriate.
No Guarantee of Treatment
You understand that completing an intake form, purchasing a program, or participating in a telehealth consultation does not guarantee a diagnosis, prescription, medication, or specific treatment.
Patient Information
You agree to provide complete and accurate information concerning your medical history, current medications, allergies, prior diagnoses, laboratory results, symptoms, and other information requested by your healthcare provider. You understand that providing incomplete or inaccurate information may affect the provider's ability to safely evaluate and treat you.
Limitations and Risks
You understand that telehealth has potential limitations and risks, including technological failures, interruptions in communication, limitations in the ability to perform a physical examination, and circumstances in which information available through telehealth may be insufficient to make a clinical determination. The healthcare provider may determine that an in-person examination, additional laboratory testing, diagnostic testing, specialist evaluation, or other medical care is necessary.
Privacy and Security
You understand that reasonable measures are used to protect information transmitted through telehealth technologies, but no electronic communication system can be guaranteed to be completely secure.
Medications
If the healthcare provider determines that prescription medication is appropriate, the provider may issue a prescription to an applicable independent pharmacy. You understand that Atlas HRT does not independently prescribe, dispense, or determine the medical appropriateness of prescription medication.
Emergency Care
Telehealth services are not intended for medical emergencies. If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
Voluntary Consent
You voluntarily consent to receive healthcare services through telehealth technology and understand that you may ask questions regarding the telehealth process, risks, benefits, alternatives, and limitations. By electronically acknowledging this Consent, you confirm that you have read and understood the information above and consent to the use of telehealth for services provided by the independent healthcare provider.
