Who We Are
Atlas HRT is a healthcare coordination platform that facilitates access to licensed healthcare providers and pharmacy services. Medical evaluations, diagnoses, treatment decisions, and prescriptions are provided solely by independently licensed healthcare professionals.
Uses and Disclosures of Health Information
Atlas HRT and the independent healthcare providers it coordinates with may use or disclose Protected Health Information for the following purposes:
Treatment
To assist healthcare providers involved in your care, including reviewing medical history, laboratory results, treatment plans, and prescriptions.
Payment
To process payments, verify eligibility, and facilitate billing-related activities.
Healthcare Operations
To conduct quality assurance activities, compliance reviews, credential verification, customer support, and other healthcare operations.
Legal Requirements
When required by federal, state, or local law, court order, subpoena, regulatory inquiry, or governmental request.
Public Health and Safety
When necessary to prevent a serious threat to health or safety or as otherwise permitted by law.

Request access to your health records. Request corrections to inaccurate information. Request restrictions on certain disclosures. Request confidential communications. Obtain a copy of this Notice. Request an accounting of certain disclosures as permitted by law. File a privacy complaint without fear of retaliation.
Business Associates and Service Providers
Atlas HRT may work with laboratories, pharmacies, technology vendors, healthcare professionals, and other service providers who require access to information in order to provide services. These parties are expected to safeguard Protected Health Information in accordance with applicable laws and contractual obligations.
Security Practices
Atlas HRT employs safeguards that may include:
Encrypted data transmission where applicable. Secure access controls. Password-protected systems. Restricted workforce access. Ongoing security monitoring and compliance procedures.
While reasonable safeguards are utilized, no method of electronic storage or transmission is completely secure.
