What causes erectile dysfunction?
ED often has more than one cause. The physician looks at all of them before deciding on any medication.
Blood flow and the heart
An erection depends on healthy blood vessels. High blood pressure, high cholesterol, smoking and heart disease can all affect them, which is one reason the physician reviews heart health first.
Hormones and metabolism
Diabetes, excess weight and low testosterone can each play a part. The physician may suggest labs, or a separate conversation about testosterone therapy.
Medicines, alcohol and smoking
Some blood pressure medicines, antidepressants and other drugs can contribute, as can heavy drinking and smoking. Never stop a prescribed medicine without talking to the doctor who prescribed it.
Stress, anxiety and relationships
Performance anxiety, stress, low mood and relationship strain can cause ED or make it worse. A PDE5 inhibitor does not treat these causes, so the physician may suggest other support alongside or instead of medication.

- Use nitrates for chest pain, such as nitroglycerin, or poppers
- Take riociguat or another guanylate cyclase stimulator
- Have been told your heart is not healthy enough for sex
- Are allergic to sildenafil or tadalafil, or take another PDE5 inhibitor, including one for pulmonary arterial hypertension
Get medical help right away for
- An erection lasting more than four hours
- Sudden loss of vision in one or both eyes
- Sudden decrease or loss of hearing
- Chest pain, dizziness or nausea during sex
