Atlas HRT's Role
Atlas HRT is a telehealth coordination and healthcare technology platform. Atlas HRT facilitates access to independently licensed healthcare providers and related healthcare services. Atlas HRT is not itself a medical provider, does not diagnose medical conditions, does not prescribe medications, and does not dispense medications.
Independent Medical Providers
Medical evaluations, diagnoses, treatment recommendations, and prescriptions are provided by independent healthcare professionals who are appropriately licensed to practice in the applicable jurisdiction. Atlas HRT does not control or direct the independent provider's clinical judgment. The independent healthcare provider determines whether a patient is appropriate for treatment, what evaluation or laboratory testing may be necessary, and whether a prescription or treatment plan is medically appropriate.
No Guarantee of Treatment
Submitting an intake form, purchasing a program, scheduling a consultation, or communicating with Atlas HRT does not guarantee that you will receive a prescription or particular treatment.
Patient Responsibilities
You agree to provide accurate and complete information regarding your medical history, medications, allergies, laboratory results, current symptoms, and other information requested during the intake and evaluation process. You are responsible for following the instructions provided by your healthcare provider and pharmacy and for completing any recommended follow-up appointments, laboratory testing, or monitoring.
Emergency Care
You should not use Atlas HRT services for emergency medical care. If you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.
Medications and Pharmacies
When a healthcare provider determines that prescription medication is appropriate, the prescription may be transmitted to an independent pharmacy for fulfillment. Atlas HRT does not manufacture, compound, dispense, or independently determine the use or dosage of prescription medications. Medication availability, fulfillment, shipping, and dispensing are subject to the policies and requirements of the applicable pharmacy.
